John Herbert "Jack" Church - Born May 24, 1915 in Kamsack, Saskatchewan – Died January 5, 1996 was a Canadian ice Hockey defenceman.

Church made his NHL debut with the Toronto Maple Leafs on November 3, 1938 vs Boston Bruins at Maple Leaf Gardens in a 3-2 Boston win.

Church scored his 1st NHL goal vs Tiny Thompson of the Detroit Red Wings at 19:22 of the 2nd period on November 18, 1939 at Maple Leaf Gardens in a 3-0 Toronto win. His goal was the game winner.

Church played 142 regular season NHL games, scoring 4 goals (2 GWG), 19 assists and 20 NHL playoff games, scoring 1 goal, 1 assist.
